Wound Care At Home

What is At Home Wound Care?

Home health wound care provides treatment for persistent and complicated wounds within the familiar environment of a patient’s home. Medicare, and some private insurance plans usually cover this care for qualifying individuals.

Expert wound care is a critical step in the healing process for any acute or chronic medical condition. Ongoing assessment of the wound and early identification of obstacles that may affect healing are crucial to ensuring proper wound closure.

Wound Care Services At Home

Our comprehensive team collaboratively manage wounds and the underlying conditions that contribute to them. These services are provided wherever you call home, be it a house, an apartment, a family member’s residence, or a senior living community.

Surgical wounds

Surgical wounds may be caused by minor outpatient procedures, complex surgeries requiring hospital admission, and everything in between.

Pressure Ulcers/Sores

Pressure injury occurs due to localized damage to the skin and underlying soft tissue due to prolonged pressure and stress.

Diabetic wounds & neuropathic wounds

Diabetic foot ulcers are often seen with conditions affecting sensation of the distal extremities (neuropathy), particularly the feet. Poor or absent sensation coupled with repetitive trauma to an area may lead to sores and complex wounds.

Trauma

Traumatic wounds are caused by direct injury to the skin and underlying soft tissue from a traumatic event such as a fall or a car accident.

Burns

Burn wounds occur from excess heat on the skin leading to skin and soft tissue damage. While scalding water and fire are often the most common causes, heat damage may also occur from less well-known sources (e.g., electricity, radiation, chemicals, friction).

Slow-healing wounds

Chronic wounds are wounds that can develop from any of the mentioned causes but may experience difficult healing due to poor blood flow and chronic inflammation to the wound area.

StepOne  Health Services PC has been a blessing!  We made an in-home appointment to have my husband’s catheter changed monthly.  My husband has limited mobility and it is difficult to get him to and from his monthly doctor’s appointment to have his catheter changed.  Thank you for making our lives a little easier each month.  We are grateful we heard about StepOne Health Services!

I went to a kidney specialist who wanted me to take follow up labs. Since his office is 40 minutes away, I called StepOne Health Services and they were able to come to my home, when it was convenient for me, and drew my blood. The results were posted on their Patient Portal and I was able to access them easily and send to my doctor. I highly recommend StepOne for in-home lab draws for anyone who leads a busy life. They were able to come to my home at a time that was convenient for me.

My elderly mom went for her checkup at the doctor’s office and wanted her annual flu shot but the doctor’s office was out of the vaccines.  He suggested going to the pharmacy.  We went to the pharmacy and was told they had not come in yet, but were expecting them any day.   My neighbor told me about StepOne so I decided to give them a call.   They came over to the house at a time that worked for us and gave Mom her shot.   Thank you StepOne that was easy!

Our son was playing with some friends and was his leg was injured in a freak accident.  The only appointment for follow up wound care we could get was during school hours.  We knew that StepOne Health Services provides wound care and made an appointment for that evening. The wound care nurse was very knowledgeable and made my son feel at ease.  Subsequent wound care visits were made and we are happy to report that the wound healed without any further issues.
